AUTHORIZATION and RELEASE for USE of TESTIMONIALS and PICTURES
I irrevocably grant the right and license to use, and to grant to others to use, publish, and/or
distribute my name, likeness, photographs of myself and/or my dog/cat, statements, testimonials
(“Testimonials”) in any manner and in all media for the following purposes:

I grant the rights set forth in this authorization and release in perpetuity and on a worldwide,
royalty-free basis:
The right to use the Testimonials in any medium, including, without limitation, print, magazines,
World Wide Web and in any media hereafter developed. This includes the right to publish, film,
produce, reproduce, digitize, adapt, perform, edit, modify, distribute, transmit, broadcast and/or
otherwise use or reuse the Testimonials in connection with Pets’ Spark.
I also agree that further prior approval to use the Testimonials in connection with the purposes
stated in this Authorization is not required.
